Support services for individuals living with symptoms of brain injury & mental health conditions

Adapta operates two separate licensed programs. Folwell Flats is the Agency’s Assisted Living option Licensed by Minnesota Department of Health with a Comprehensive Home Care License, they provide their services within their Housing with Services license. Folwell Flats is also able to take Minnesota Waiver money under the Customized Living Option.

Adapta also holds a 245D-HCBS program license and are licensed to provide Intensive Support Services. These services are provided at our two Community Residential Service sites; Manor House and Judd House. Independent Living Skills Training is also offered to individuals that fit Adapta’s mission and live in their own home or other community sites.

Regardless of setting, individuals served can expect a program tailored to their specific needs and desires.  Adapta has emerged as a leader in community integration. Their unique design allows for multiple group and individual outings per day.

Here are some things you can expect at Adapta:

On-site:

  • 20+ hours of on-site RN nursing per week
  • On-site Social Worker
  • Community Integration Coordinator
  • Overnight awake staff
  • Individualized goals and programming (emotional/behavioral, chemical, and physical health)
  • Recreational activities and programs
  • Case coordination via Registered Nurse, Social Worker, and Community Integration Coordinator
  • Weekly AA (when needed),
  • Monthly music, art and pet therapy
  • Twice monthly psychological services through Zumbro Valley Mental Health Center
  • Wi-Fi/cable in rooms and common areas
  • Private rooms fully furnished including beds, linens, comforter, closet and end table
  • Medication management
  • Three catered meals daily plus snacks and beverages available
  • Community computers and free laundry
  • Money Management and Resident Trust Accounts available

Off-site:

  • Access to 24-hour nursing (via phone)
  • Transportation and coordination with community resources
    • Individual community outings
    • Large group outings
    • AA/NA/Treatment groups
    • Coordination with local employers and day treatment programs
